What Is An Enrollment Center? |
|
|
An Enrollment Center is a community agency or business
that has been equipped with the materials and training to assist people in
applying for Hoosier Healthwise. Enrollment Centers are one of the three
ways in which a person can apply for Hoosier Healthwise, the other two being
at a Division of Family Resources or by completing a
mail-in-application. Enrollment Center personnel offer help in completing
the application and basic information on the Hoosier Healthwise Health Care
Program. Once the application has been completed, it is then forwarded to
the Division of Family Children Office where the eligibility is determined.

In order to become an Enrollment Center for the Hoosier
Healthwise Program, please contact your local Division of Family Resources. |

I n order to discontinue Enrollment Center functions within your organization, please contact your local Division of Family Resources. |

There are no time restrictions placed on Enrollment
Centers. Each Enrollment Center must sign a new Memorandum of Agreement each
year in order to remain active, but can quit at anytime. |

I n order to receive additional Hoosier Healthwise Applications and supplement sheets, you can contact your local Division of Family Resources Office or you can order them from the Distribution Center. Draft a letter (on your agency stationery) to the Forms Distribution Center at the following address:Department of Administration Make sure you include in the letter: * The number of applications you are ordering (in quantities of 100). * The form number: Hoosier Healthwise (purple) application form (English) - State Form 43202 Hoosier Healthwise (purple) application form (Spanish) - State Form 48974 * The name of a contact person in your agency and phone number in case the distribution center needs to contact you. * If you need to contact the Distribution Center, the number is (317) 591-5228. |

It is imperative that all individuals that are applying for Hoosier Healthwise contact their local Benefit Advocate or
1-800-889-9949 to speak with a Benefit Advocate to select their Primary
Medical Provider (PMP). This selection will prevent the member from being
auto-assigned to a PMP that they do not wish to see if and when they are
determined eligible for Hoosier Healthwise. |

The Division of Family Resources receives all of the
completed applications and eligibility is determined there. If a person does
not agree with the decision, he or she should talk with his or her case
worker in order to determine the reason he or she was denied coverage and
for information on the appeals process. Everyone has the right to appeal an
eligibility decision and the caseworker can inform him or her on the proper
process. |
